Healthy, thriving communities

Our community health assessment and planning work identifies the strengths and needs of a community. These activities inform funding decisions, prioritize program development areas, and identify implementation strategies, all with the goal of improving the health of residents.

While we offer many assessment types and plans, our approach is asset-based and community-centered. HRiA provides a comprehensive suite of services that integrates current models and best practices in the field and meets or exceeds compliance requirements. We customize each project to address the needs of clients and communities.

Areas of Expertise

Community Health Assessment

Uncovering community health needs and assets.

Assessment studies can support regulatory compliance, establish good public health practices, and improve the health of residents. We offer a range of options including Community Health Needs Assessments for accreditation and regulatory requirements, and Health Impact Assessments, which evaluate the potential health effects of a project, policy, or plan on a community. Also, our Topical Assessments examine specific health and/or social conditions in a particular population or geography.

Community Health Improvement

Bettering community health through policy, systems, and environmental change.

Community Health Improvement integrates assessment and planning to support regulatory compliance, strengthen collaboration, and improve community health outcomes. We develop Community Health Improvement Plans and Strategic Implementation Plans that align priorities, translate data into action, and help hospitals, health systems, community health centers, health departments, and other partners meet regulatory and accreditation requirements (e.g., IRS Form 990, PHAB accreditation, etc.).

Formative Research

Designing effective health initiatives.

We refer to activities informing the development of an initiative as formative research. These activities match client needs and goals. They may include environmental scans, landscape analyses, literature reviews, initial key informant interviews, and feasibility studies.

Racial Equity Assessment

Understanding organizational capacity for change.

Racial equity assessments provide in-depth information about organizational capacity to advance equity by transforming culture, policies, practices, and systems. We identify key factors that may facilitate or impede the success of equity efforts, including staff knowledge, attitudes, and experiences. Assessment results provide baseline data as a strategy for knowing where to focus and to measure progress on organizational efforts to advance equity.

Strategic Planning

Developing actionable strategic plans to help organizations achieve their goals.

Strategic planning is a cornerstone of organizational excellence. We work with organizations to identify priorities and develop goals, measurable objectives, and strategies for a strategic plan that inspires and guides action and results. We tailor our strategic planning services to meet your needs and ensure meaningful participant engagement and stakeholder input on all elements of the plan. If you are not ready for a full strategic planning process, we also facilitate strategic discussions, convene and engage key stakeholders, and/or conduct prioritization processes to help inform future or ongoing planning efforts.

Together with HRiA, they pursued a joint community health assessment and plan. Advancing health equity at the local level

Lincoln County Health and Human Services (HHS) in Oregon partnered with HRiA to advance equity within their department. Grounded in HRiA’s Health Equity Framework, this work aligned Lincoln County HHS’s culture and practices with its goals of positive, equitable health outcomes for its community. Representatives from the team went on to present their findings at the 2023 NACCHO 360 conference. Join us on the journey from local health department to national stage in our impact story.
